[Wannier] Regarding 'surface card' for input card

Gretings!!

I'm Debasish, PhD student from IIT Jodhpur, India. I want do some
calculation for which I need the 'surface card' for the input file, which
looks like this in the manual.

SURFACE            ! See doc for details
 1  0  0           a11, a12, a13
 0  1  0           a21 a22 a23

I have gone through the manual, but did not understand how to construct
this card. I have searched a lot about this, but got nothing.

Can this be obtained, if the lattice type and the hkl plane is known?
Please help me find the way ut construct this surface card.

Kindly help me out in this regard.
