This question has been discussed several times in the forum. You could find the answer if searched the forum archive.

Nevertheless, simply change max_irr_dim = 4 to some reasonable value (6 or 8, say) in phcom.f90 in PH directory. And recompile phonon code.
This will fix the problem.
